About the Role As a full-time remote Social Worker, you'll play a vital role in our DSNP care management team, working closely with Care Managers, providers, and community organizations to address the complex health and social needs of our members. This is an exciting opportunity to make a life-changing impact on the lives of our members, who are enrolled in both Medicare and Medicaid. Key Responsibilities: Participate in care planning, including assessment, goal setting, establishing interventions, identifying barriers, and monitoring progress. Develop and implement strategies to coordinate and integrate post-acute and long-term care services, preventing exacerbations and custodial care placements. Conduct initial and periodic assessments of members enrolled in Long-Term Care Programs or care programs. Apply social work concepts, principles, and strategies to address social determinants of health needs in individualized care plans. Collaborate with providers, primary care physicians, Medical Directors, pharmacists, and care management staff to ensure comprehensive care. Serve as a member advocate, ensuring access to necessary care and community resources. Develop relationships with hospital social workers and community resources to optimize care management. Identify and report potential quality improvement issues. Direct social work interventions, including psychosocial assessments, telephone follow-arenaflex, and in-home or facility assessments. Requirements and Qualifications Essential Qualifications: Minimum of 2+ years of experience in medical social work or case management. Active and unrestricted LMSW licensure in the state of residence. Master's Degree in Social Work required. Preferred Qualifications: Medicare and Medicaid experience. Care Coordination experience.
